29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about 02/15/15 in the injection mold area; employer failed to protect employee(s) involved in service and maintenance activities, such as but not limited to, mold changing on the Cincinnati injection mold machine, from caught between hazards when they allowed the use of the emergency stop button and the access door safety switch as energy isolation and did not conduct training on controlling hazardous energy during service and maintenance work for employees involved in these activities.    b) On or about 02/15/15 in the surface grinder room; employer failed to protect employees involved in changing the grinding wheels on the Mitsui and MHT brand surface grinders ( which were hard wired to the power sources ), from caught between hazards, when they allowed the use of the stop button as energy isolation and did not conduct training on controlling hazardous energy during service and maintenance work  for employees involved in these activities.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) On or about 02/05/15 in the grinder room; employees were exposed to the inhalation of a hazardous chemical to clean parts, acetone ( flammable liquid ), and the employer did not maintain a develop or maintain a written hazard communication program.      b) On or about 02/05/15 in the de buring area; employees were exposed to the inhalation of a hazardous substance when they de burr aluminum parts ( skin and eye irritation and combustible dust  ) and the employer did not develop or maintain a written hazard communication program.      ABATEMENT CERTIFICATION REQUIRED

(a) On or about 02/05/15 in the grinder room; employees used a hazardous chemical to clean parts, acetone ( flammable liquid ), and the employer did not maintain a safety data sheet for this chemical.         b) On or about 02/05/15 in the deburing area; employees were exposed to a hazardous dust when they de burred aluminum parts ( skin and eye irritation ) and the employer did not maintain a safety data sheet for this material.
