The hole measured approximately 4 feet wide and 8 feet long, with a fall distance of approximately 10 feet. Observed on or about 2/5/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section. a) Jobsite: 2nd Level - No means of fall protection was used to protect employees from falling off the unprotected open sides and edges of the residential construction building. The employees were performing framing operations approximately 10 feet above ground level. Observed on or about 2/5/15. Alexander construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 933986, citation number 1, item number 2a, and was affirmed as a final order on 4/08/14, with respect to a workplace located at 512 South Front Street, Philadelphia, PA 19147.
