On or about February 11, 2015, west side of building:  The exit route from the emergency exit on the west side of the building was covered with accumulated ice and snow.  The exit door was unable to open fully and the stairs and path had not been cleared.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  A.  On or about February 11, 2015, in the following locations, exit routes were not kept free and unobstructed:  1.  South side of building:  Two filled propane cylinders were stored in the exit route from the designated emergency exit.  2.  West side of building:   Pallets of materials were stacked three-high, totally obscuring the designated emergency exit.  3.  West side of building:  Ice and snow buildup outside the building prevented the emergency exit door from opening.

The lifting chart on the Mitsubishi powered industrial truck was illegible, as it was coated with grease.

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less:  A.  On or about February 11, 2015, Warehouse, at the Electrical Panels:  There were no points of entry to the working space about the main breaker and electrical panel which powered heat and air conditioning units.  The storage of wire reels blocked access to the electrical equipment.
