On January 28, 2015 an employee sustained a crushed hand and three finger amputation when he reached in to clear a jam on the 01C Danly Mechanical OBI Power Press and another employee cycled the press.

Employees are required to perform hand in die operations while the 01C press was in INCH mode, bypassing all safety features, including, but not limited to the brake monitor, and anti-repeat features, and the 01D press bypassing the top stop faults, exposing them to amputation hazards.
