Shiloh Industries, Inc. dba Wellington Stamping located at 350 Maple St. Wellington, OH 44090; The employer did not assure the fixing of selection for the Off, Inch, Single Stroke, and Continuous modes on the OBI Danly Mechanical Power Presses were only by means capable of supervision by the employer. Employees are required to change the modes of the presses for adjusting the top stop and to run production and are exposed to amputation hazards in that no key is required to change modes.

Employees are required to perform hand in die operations while the 01C press was in INCH mode, bypassing all safety features, including, but not limited to the brake monitor, and anti-repeat features, and the 01D press bypassing the top stop faults, exposing them to amputation hazards.

On January 28, 2015, an employee reached into the press and sustained a 3 finger amputation when a second employee inadvertently cycled the press at the same time.
