29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board: (a) On or about February 06, 2015 - Wheel Hub Cutter, Line #1 and #2, employees are exposed to falls onto, in between, and off of a metal post between the conveyor system and the chute opening while cleaning the dross from the wheel hub chute walls.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard are being followed: Hub Cutter Area; the employer did not fully document its periodic inspections, at least annually, of the energy control procedures on HW 18-1 Conveyor Hub Cutter #1 and HW 22-1 Conveyor Hub Cutter #2 to ensure that the procedure and the requirements of the Lockout/Tag out standard were being followed.
