Establishment - On or about February 4, 2014, An employee, maintenance worker, iwas exposed to hazardous chemicals such as 2- Butanone(MEK) and flammable inks during the maintenance of the Hitachi PXR Inkjet printer. The employer had not developed and implemented a program as required.
