29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a. On or before January 30, 2015, employees hoisted turf maintenance equipment, used a chain sling without an inspection of the chain being conducted, thereby exposing employees to a struck-by hazard. b. On or before January 30, 2015, employees hoisted turf maintenance equipment, using a hook that was missing it's safety latch, without an inspection of the hook being conducted, thereby exposing employees to a struck-by hazard.

On or before January 30, 2015, employees used an alloy steel chain without any permanently affixed durable identification stating size, grade, rated capacity, and reach, to hoist turf maintenance equipment, thereby exposing employees to struck-by hazards.
