29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition: a. On or about February 6, 2015, owner and supervisor operated the double insulated DEWALT model: DW368 portable 7-1/4-inch circular saw serial: 185324 that had not been maintained in a safe condition to cut wood. The owner had cut the power cord and repaired with inadequate insulation i.e. electrical tape, being exposed to electric shock hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal arrest systems, or a safety monitoring system: a. On or about February 6, 2015, supervisor and an employee installed replacement wood on the low slope roof of the porch area without the use of any fall protection systems, being exposed to a 7-foot 9-inch fall hazard.
