29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Storage Cage; A relocatable power tap was plugged into an extension cord which powered a computer inventory system. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate: a) Storage Area; Three fiberglass step ladders and a box were stored on top of a wire mesh storeroom. The storage room stored automotive tools and parts. There was no loading rating on the storage room structure, thus it could collapse if the items stored above were too heavy.
