29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Front porch roof, new residential construction located at 151 Nakomis Place, Milton, GA 30004: Employee was exposed to eye injuries while operating a Hitachi (Model - NT65M2S) 2-1/2-inch Finish Nail Gun at 120 PSI without protective eyewear.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Front porch roof, new residential construction located at 151 Nakomis Place, Milton, GA 30004. Employee was exposed to falls of approximately 18-feet while installing Hardie-Plank Siding from a Ladder Jack Scaffold which was erected on a porch roof with no connection to the fall protection system provided. The employee was wearing a fully body harness and failed to connect to the life-line anchored to the roof.
