The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.      a) On or about December 12, 2014, a customer service representative, operating the shredder in truck #878, received injuries to their right forearm when their cuff became caught by the shredder knife blades and the employee was pulled into the shredder. The shredder's point of operation was not guarded exposing the employee to amputation hazards.       b) At the facility, the point of operation of the shredder in truck #1382 was not guarded exposing employees to amputation hazards.        c) At the facility, the point of operation of the shredder in truck #238 was not guarded exposing employees to amputation hazards.

When there is occupational exposure, the employer shall provide, at no cost to the employee, appropriate personal protective equipment such as, but not limited to, gloves, gowns, laboratory coats, face shields or masks and eye protection, and mouthpieces, resuscitation bags, pocket masks, or other ventilation devices. Personal protective equipment will be considered "appropriate" only if it does not permit blood or other potentially infectious materials to pass through to or reach the employee's work clothes, street clothes, undergarments, skin, eyes, mouth, or other mucous membranes under normal conditions of use and for the duration of time which the protective equipment will be used.  a) At the establishment, Customer Service Representatives (CSR) performing shredding services at facilities such as, but not limited to, hospitals, doctors' offices, dental offices, and other health care providing facilities used their hands to remove any remaining documents after dumping the contents of totes and bags into the shredder's hopper. The CSRs were not protected from sticks from contaminated sharps by being provided appropriate hand protection exposing them to the hazard of blood-borne diseases.
