Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On 6 Feb, 2015, three of the four employees of Caen Hernandez were observed working on a residential roof located at 811 Russell Blvd, Saint Louis, MO. All employees on the roof were engaged in tear off of layers of roofing material while on top of the building that measured 30 feet at the eave and an additional 10 feet to the peak with a 6/12 pitched slope, as verified by Caen Hernandez.

All three employees utilized the one access ladder to move take material up to the roof top throughout the day. The single access ladder did not extend 3 feet above the upper landing surface.
