29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. On 4 February, 2015, located In the break room, KAO Brands Warehouse Expansion & Storage job-site, an employee was exposed to an electrical shock while performing stripping and terminating 277V conductors for installation of the Fluorescent 2' X 4' Suspended Ceiling Troffer lights that had not been deenergized or guarded by insulation or other means.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. On 4 February, 2015, located In the break room, KAO Brands Warehouse Expansion & Storage job-site, an employee did not ascertain prior to stripping and terminating the conductors for installation of Fluorescent 2' X 4' Suspended Ceiling Troffer Lights was protected or isolated from energized 277 volts, in that no procedures were in place by the employer to evaluate conditions for possible contact with energized conductors.

29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On 4 February, 2015, the employer was notified that an employee was stripping and terminating 277V conductors for installation of Fluorescent 2' X 4' Suspended Ceiling Troffer lights received an electrical shock that required the employee to be admitted into a hospital. The employer did not report the employee in-patient hospitalization to OSHA within the twenty-four hours after the work related incident.
