29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: On or about February 6, 2015: Employees performing custom millwork used N95 masks voluntarily to protect against dust. The employer failed to provide the limitations and other information contained in Appendix D to employees prior to mask use.

29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5): On or about February 6, 2015: Employees in the assembly area used wood glue to manufacture custom cabinets and countertops. The employer failed to ensure bottles of glue were labeled appropriately.
