29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets: a) On or about 02/04/2015, carpenters were exposed to a struck-by hazard while working below others framing a roof and were not provided with head protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: a) Third Floor - On or about 02/04/2015, carpenters framing the exterior walls of a house under renovation were not provided with fall protection and exposed to a 30 foot fall. b) Roof - On or about 02/04/2015, carpenters framing the roof of a house under renovation were not provided with fall protection and exposed to a 30 foot fall. c) First Floor Stairway On or about 02/04/2015, carpenters were exposed to a 10 foot fall through an unguarded floor opening. d) Second Floor Stairway On or about 02/04/2015, carpenters were exposed to a 10 foot fall through an unguarded floor opening.
