29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 9 February 2015, and at times prior thereto, two framers performing cornice work where one was working at a roof eave while the other was providing materials or equipment above were not wearing head protection, exposing them to the hazard of being struck by an overhead object such as the roof eave or by falling tools, materials, and debris.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 9 February 2015, and at times prior thereto, a framer was performing cornice work without the use of eye protection, exposing the worker to the hazard of being struck to the face and eyes by flying objects. The framer utilized both pneumatic nail guns and a hand-held circular saw while at the first story.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage. On or about 9 February 2015, and at times prior thereto, a framer was using a hand-held circular saw to cut cornice pieces that was connected to an extension cord that had damage showing the internal wires, exposing that worker to an electrical hazard. One section of exposed wiring had a cut in it showing the copper wire. The cord was plugged into a GFCI outlet.
