29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about January 28, 2015 and at times prior thereto, employees of Ricardo Martinez, a subcontractor contracted by DDO Construction, LLC, were using a work platform that was not fully planked.  Employees were exposed to a fall up to 40 feet.

29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm)  On or about January 28, 2015, and at times prior thereto, employees of Ricardo Martinez, a subcontractor contracted by DDO Construction, LLC, were working from a scaffold of which the plank ends did not extend at least 6 inches from the scaffold support.  Employees were exposed to a fall of up to 40 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    On or about January 28, 2015 and at times prior thereto, employees of Ricardo Martinez, a subcontractor contracted by DDO Construction, LLC, did not have adequate means to access the working platform of the scaffold.  Employees were exposed to a fall of at least 40 feet.

On or about January 28, 2015, and at times prior thereto, employees of Ricardo Martinez, a subcontractor contracted by DDO Construction, LLC,  were working from a scaffold of which one side presented a fall of up to 40 feet and no fall protection was provided.  Employees were exposed to a fall of up to 40 feet.
