29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:    a)  On or about February 10, 2015 at the employer's location at 178C Corporate Drive, Gloversville, NY 1207 the employer did not ensure that employees working in the production facility used protective footwear to protect themselves from falling objects such as but not limited to heavy equipment, tools and pallets.

29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in 29 CFR 1910.6:    a)  On or about February 10, 2015 at the rear of the employer's location at 178C Corporate Drive, Gloversville, NY propane cylinders were stored and could be tampered with by an unauthorized person.  The propane cylinders were not secured in a locked enclosure.
