29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation, and when the operator did not have a clear and direct view of the edge of excavation: a. Building 252 - southwest area, JLG High Reach 2 800S aerial lift fell into an adjacent excavation. A warning system such as barricades, hand or mechanical signals, or stop logs was not used while mobile equipment was operated adjacent to the excavation. Violation observed on or about 2/10/2015.
