29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a) Allied Insulation at 1152 S. Ogden Street, Denver, CO 80210: On and before 2/6/15, the employer failed to ensure that employees engaged in gutter installation activities 6 feet or more above lower levels were protected from falling by an adequate fall protective measures. The condition exposed employees performing work to fall hazards of approximately 20 feet at the eave. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladder s length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder: (a) Allied Insulation at 1152 S. Ogden Street, Denver, CO 80210: On and before 2/6/15, the employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employee was using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 10 feet fall while attempting to use the ladder. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
