29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:    On or about February 10, 2015 and times prior thereto, at a worksite located at or near 103 Quapaw Trail, Maumelle, AR, employees were observed on the north side of the work site cutting copper sheet metal with electric powered shears that had cord damage exposing employees to electric shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about February 10, 2015 and times prior thereto, at a worksite located at or near 103 Quapaw Trail, Maumelle, AR, employees were observed on the north side of the worksite cutting copper sheet metal with electric powered shears that had cord damage and plugged into an extension cord that was damaged and had insulated wires exposed.

29 CFR 1926.1053(b)(1): Where portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.        On or about February 10, 2015 and times prior thereto, at a worksite located at or near 103 Quapaw Trail, Maumelle, AR, employees were observed descending 20 feet on an extension ladder  that did not extend 3 feet above the roof and did not have a grab bar to assist employees during mounting and dismounting the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      The C. Cougill Roofing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number315174433, citation number 1, item number 1 and was affirmed as a final order on 5-17-2011, with respect to a workplace located at 2803 Shenandoah Valley Drive, Little Rock, AR.    On or about February 10, 2015 and times prior thereto, at a worksite located at or near 103 Quapaw Trail, Maumelle, AR, employees were observed performing roofing activities on the north side roof of the residence at a height greater than 20' from the concrete ground below. Workers were not using fall protection or any alternate form of fall protection.
