Zurn Industries, LLC, 1801 Pittsburgh Avenue, Erie, PA 16502, on or about February 6, 2015 and times prior thereto:  Employees perform lockout/tagout on equipment including, but not limited to, the Hitachi Seiki 2201 and 2202 Vertical CNC machines, for service and maintenance type purposes that include, but are not limited to, preventative maintenance and repairs.  Specific procedural steps were not noted on the lockout/tagout procedure for pneumatic energy isolation, for the verification of pneumatic energy source isolation, and for the specific techniques for bleeding out the air to bring the door to a zero-energy state.  In addition, the lockout tagout procedure did not include specific procedures / techniques for blocking the stored energy of the door when work necessitated the door to be in a raised position.    B.)  Zurn Industries, LLC, 1801 Pittsburgh Avenue, Erie, PA 16502, on or about February 6, 2015 and times prior thereto:  Employees perform lockout/tagout on equipment including, but not limited to, a Bridgeport Torq-Cut VMC, a Cincinnati Sabre 500 and a Cincinnati Sabre 1000 VMC, for service and maintenance type purposes that include, but are not limited to, preventative maintenance and repairs.  Specific procedural steps were not noted on the lockout/tagout procedure for pneumatic energy isolation, release of residual pneumatic energy, and for the verification of isolation for pneumatic energy sources.     C.)  Zurn Industries, LLC, 1801 Pittsburgh Avenue, Erie, PA 16502, on or about February 6, 2015 and times prior thereto:  Employees perform lockout/tagout on equipment including, but not limited to, a MSK Machine, for service and maintenance type purposes that include, but are not limited to, preventative maintenance and repairs.
