OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible aluminum powder deflagration hazards while manually transferring combustible aluminum powder from a portable container and into a processing vessel in a manner in which a combustible dust deflagration was possible: On or about February 6, 2015, the CT-41 line's heating mixer vessel was loaded with a mixture of aluminum powder (including stabilizing agents) and other materials, such as but not limited to, resin. The aluminum powder mixture was manually transferred/dumped into the mixer's metal receiving hopper from a metal bucket in a manner that presented the necessary elements to support a combustible metal dust deflagration. The powder mixture formed a dense dust cloud in the presence of oxygen and a suitable ignition source. Recognized means of equipment and personnel bonding/grounding were not utilized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to the hazard of being caught in ingoing nip points on extruding machine, MPC EX-55. The chiller rollers were not guarded to protect employees from such injury. This item was corrected during inspection.

29 CFR 1910.305(g)(1)(i): Flexible cords and cables were not approved for conditions of use and location: A flexible cord, on CT-41 mixer was not approved for the condition of use and location. The cable was damaged and there were exposed conductors. Also the cable was located on equipment which was in a classified hazardous location. Employees were exposed to fire/burn hazards and injury. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

The following equipment was located in this area and was not intrinsically safe, approved for the hazardous (classified) location or safe or the hazardous (classified) location: a. Lighting fixtures located approximately 105 inches from CT-41 and approximately 92 inches from CT-42. b. Two crane and hoist systems, each located approximately 80 inches directly above CT-41 and CT-42. c. Circuit boxes, which services Crane #1 and Crane #2, located approximately 105 inches from CT-41. d. An outlet installation located approximately 92 inches from CT-42. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
