OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to being struck-by falling material(s):  (a) On or about February 10, 2015 - Tooling and Fabrication Department, an employee performing fabrication work propped up a 90 inch, 200 pound piece of square tubing using only 3/16 th of an inch lip under the side edge of a Hyd-Mech Vertical Band Saw, model V-18, resulting in the partial fingertip amputation of the thumb.
