29 CFR 1910.27(d)(2)(i): Where a man had to step a distance greater than 12 inches from the centerline of the rung of a ladder to the nearest edge of structure or equipment, a landing platform was not provided: Employees are exposed to fall hazards when they access the fixed ladder of the GSI Portable Dryer Unit to a height greater than 16 feet from the ground below without a landing platform for safe access for employees to perform maintenance of the portable dryer unit.

29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off. Employees are exposed to caught-in hazards when entering the grain bins to level the rice for proper air flow. The auger is not de-energized prior to employees entry into the grain bin.
