29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): 1.) On or about February 10, 2015, the employer failed to ensure employees who were required to wear a dust mask during plasma cutting operations were fit tested to determine if the respirator seal was adequate. Employees were exposed to hazards from iron oxide plasma cutting operations.
