29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    Worksite located at 1441 NW 24th Ave. Norman, OK 73069. On or about February 9, 2015 and at times prior thereto, employees were observed performing work from a scaffolding platform that was not fully planked between the front and rear uprights.  The employees were exposed to a fall hazard of approximately 18 feet.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  Worksite located at 1441 NW 24th Ave. Norman, OK 73069. On or about February 9, 2015 and at times prior thereto, employees were observed working from scaffolding where the end of the platform extended over its support more than 18 inches and did not have guardrails that blocked employee access to the cantilivered end. The employees were exposed to a fall hazard of approximately 18 feet.
