29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) Throughout facility: Ventura Foods did not provide machine specific lockout tagout procedures for the main air shut off valves for the Portion Pack Room at the north side of the deck. On or about February 8, 2015 a Production Maintenance Lead was injured when trouble shooting the main air valves in the Portion Pack Room.
