29 CFR 1926.154(a)(1): Fresh air shall be supplied in sufficient quantities to maintain the health and safety of workmen. Where natural means of fresh air supply is inadequate, mechanical ventilation shall be provided.     a. At building 41 where employees used a kerosene forced air heater to heat the interior of the apartment, the apartment was closed and no means for fresh air was provided, thereby exposing employees to a carbon monoxide exposure Hazard.

Suitability of equipment for an identified purpose may be evidenced by listing, labeling, or certification for that identified purpose.    a. At the worksite where employees used a job made extension cord, a metal enclosure box was used, not in accordance with its listed and labeled use, thereby exposing employees to an electrical contact hazard.

At the worksite where employees used an extension cord to provide power to a heater, the ground prong was missing from the cord, thereby exposing the employees to an electrical contact hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.    a. On the second floor of building 41 where employees were using an extension cord to provide power to a heater, the extension cord was ran through the sliding glass door without protection, thereby exposing employees to an electrical contact  hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.    a. At building 41 where employees used 2 inch by 6 inch boards to access one balcony to another, no fall protection measures were used, thereby exposing the employees to an 11 foot 8 inch fall hazard.
