29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:    a)  At the establishment, on or about 2/12/15:  The bloodborne pathogens exposure control plan was not reviewed annually to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure.  The plan also did not reflect job classifications for potential exposures, who the first responders were, and what appropriate disinfectant was to be used and how to use it. 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:    a)  CNC Machining Area, on or about 2/12/15:  Rotating spindle end and nut projections on a Dayton buffing/deburring wheel next to Haas VMC #3 were not guarded.    b)  CNC Machining Area, on or about 2/12/15:  Rotating spindle end and nut projections on an 8 inch buffing/deburring wheel next to Haas VMC #2 were not guarded.    c)  CNC Machining Area, on or about 2/12/15:  Rotating spindle end and nut projections on a Central Machinery buffing/deburring wheel next to Haas VMC #5 were not guarded.    Specialty Welding & Fabricating of NY Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.215(a)(2), which was contained in OSHA inspection number 623718, Citation 1, Item 2a and was affirmed as a final order on 11/21/12, with respect to a workplace located at 1025 Hiawatha Blvd. East, Syracuse, New York.    Abatement documentation must be submitted for these items.
