29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights     a) On February 6, 2015 and at times thereto; Front of building; employees had erected a single pole scaffold which was not completely planked exposing them to a 30 foot fall to ground level.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:  On February 6, 2015 and at times prior thereto; front area of building; employees working a scaffold system were not protected by both a guard rail system and a full body harness exposing them to an estimated 30 foot fall to ground level.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On February 6, 2015 and at times prior thereto; throughout facility; employees framing a new residential Condo were observed walking on open platforms without fall protection in place to prevent them from falling 35 feet to ground level.       b) On February 6, 2015 and at times prior thereto; third floor front side; employees were walking through or working near wall openings without fall protection in place exposing them to 35 foot fall to ground level.
