29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. (a) In the storage room, FedEx boxes of supplies and metal racks extended beyond the top shelves of the gray metal storage cabinets. (b) In the locker rooms, brown boxes of materials, coffee maker and swiveling chair were stored on top of lockers. (c) In the I.T. room, there were video display terminals and other materials stored on the top shelf of metal racks.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. (a) At the FLEO Chief's office, a re-locatable power tap was plugged into a black extension cord to extend the length of the cord. (b) At the District Asset Forfeiture Coordinator's office, re-locatable power taps were daisy chained, one re-locatable power tap was connected to the other. (c) At the AO's office, re-locatable power taps were connected in series to the other.
