29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  At the establishment employees performing maintenance and preventive maintenance on hard wired equipment such as the variable frequency drive for the secondary chill water pumps, air conditioning systems, and etc., were exposed to electrical hazards due to the written lockout / tagout program missing the following elements:  a)  The training section did not address specific training requirements for qualified, unqualified, affected, and other employees at the facility.    b)  The training section did not also address the conditions for retraining employees.  c)  The written program did not address an enforcement policy for violating the lockout / tagout program.  d)  The written program did not address performing periodic inspections of the lockout / tagout program.  e)  The written program did not address the application of control measures (lockout / tagout devices).  f)  The written program did not address specific procedures for equipment with multiple energy sources.  g)  The written program did not address group lockout / tagout.  h)  The written program did not address the requirements for contractors performing lockout / tagout at the facility.
