US Interstate Highway 90 East, Auburn, MA: On or about February 11,2015: employer failed to report a work-related fatality within 8 hours of the incident which occurred at 2:57 PM February 11, 2015 and was media referred to the Springfield Area Office at 9:55 AM February 12, 2015.
