29 CFR 1926.453(b)(2)(iii): Employee(s) were not prohibited from belting off to an adjacent pole, structure, or equipment while working from an aerial lift:  The employer is failing to protect the employees from fall hazards associated with simultaneously tying off to the inside of the aerial lift basket and to an adjacent structure.  The most recent example of this was found on the job site located at Cell tower behind 5545 Cornhusker Hwy, Lincoln, NE 68507where the employer has two employees wearing fall arrest harnesses with dual 6 lanyards, using a JLG 1350SJP Aerial Lift to make repairs and upgrades to cellular antennas mounted on a cellular tower and in the course of their work the employee had one of his two lanyards attached at the approved attachment point on the basket then secured the second lanyard to an adjacent antenna bracket that was not rated for fall protection thus exposing the employee to being pulled from the basket and suspended between the lift and the antenna in the event of a mechanical failure of the lift or if the operator were to move the basket without the other employee disconnecting first.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  The employer is failing to protect the employees from fall hazards associated with climbing on and leaning over the guard railing of the basket of the aerial lift.  The most recent example of this was found on the job site located at Cell tower behind 5545 Cornhusker Hwy, Lincoln, NE 68507where the employer has two employees wearing fall arrest harnesses with dual 6 lanyards, using a JLG 1350SJP Aerial Lift to make repairs and upgrades to cellular antennas mounted on a cellular tower and in the course of their work were climbing onto the mid-railing to assist them in reaching spots on the tower that they were unable to reach while standing on the basket deck.
