29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete: a) On or about February 18, 2015, at the above located workplace, the employer's 2014 OSHA 300A form was not completed in detail as required by 1904.4 through 1904.32. In that: i) During 2014 the employer recorded one lost work day injury on the OSHA 300 form. The OSHA 300A had zero injuries and zero lost work days. ii) The employers OSHA 300A summary not properly certified by a company executive.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written personal protective equipment (PPE) certification that identified the workplace evaluated; the person certifying that the evaluation was performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment: a) On or about February 18, 2015, at the above located workplace, employees were required to work with and around hot extruded plastic and others hazards. These hazards necessitated the use of personal protective equipment including, but not limited to protective gloves. The employer had not verified, through a written certification, that the required workplace hazard assessment had been performed.
