29 CFR 1910.1000(a)(2): An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C",  exceeded the 8-hour Time Weighted Average given for that substance in any 8-hour work shift of a 40-hour work week.    a. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee A working in the production area of the facility was exposed to carbon monoxide at a concentration of 191 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 382%.    b. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee B working in the production area of the facility was exposed to carbon monoxide at a concentration of 586 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 1172%.    c. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee C working in the production area of the facility was exposed to carbon monoxide at a concentration of 281 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 562%.    d. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee D working in the production area of the facility was exposed to carbon monoxide at a concentration of 257 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 514%.      e. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee E working in the production area of the facility was exposed to carbon monoxide at a concentration of 290 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 580%.    f. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, employee F working in the production area of the facility was exposed to carbon monoxide at a concentration of 166 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 382%.

29 CFR 1910.1000(e): To achieve compliance with paragraphs (a) through (d) of this section, administrative or engineering controls were not first determined and implemented whenever feasible. When such controls were not feasible to achieve full compliance, protective equipment or any other protective measures were not used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and/or technical measures used for this purpose must be approved for each particular use by a competent industrial hygienist or other technically qualified person. Whenever respirators are used, their use shall comply with 1910.134.  a. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, the employer did not ensure engineering controls were in place to ensure employees were not overexposed to carbon monoxide.  Employee A working in the production area of the facility was exposed to carbon monoxide at a concentration of 191 ppm as an 8-hour timed average exposure which exceeded the OSHA Permissible Exposure Limit (PEL) of 50 ppm by 382%.  b. 29 CFR 1910.1200(h)(1):  Employee training did not include at least the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area.    a. American Excelsior Company located in Norwalk, Ohio:  On or about February 2, 2015, the employer did not ensure employees were provided with information and training on the health effects of carbon monoxide.  Employees working in the production area were exposed to carbon monoxide up to 586 ppm as an 8-hour time weighted average exposure.    American Excelsior Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 910396, citation number 1, item number 3 and was affirmed as a final order on December 18, 2013, with respect to a workplace located at 180 Cleveland Rd, Norwalk, OH.
