29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning. (a) An employee had a partial finger amputation when he was cleaning a ring on the lapping machine. This activity was not included in the employer's Energy Control Program/Lockout/Tagout procedure for the lapping machine. This activity required the employee to put his hand in to an associated danger zone during the machines operating cycle. The employer provided abatement at the time of the inspection.
