29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about February 10, 2014, employees were performing maintenance, on a machining center, under lock out/tag out procedures that did not include steps for securing or blocking gravitational energy. These conditions exposed the employees to the hazard of being caught in the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 12, 2015, and prior thereto, maintenance employees were operating milling machines and drill presses that were not equipped with chuck guards. These conditions exposed the employees to the hazard of being caught in the rotating chucks in the following instances: a) The Trump milling machine b) The Clausing drill press
