The employee had not received training as an authorized employee. The employee did not possess an adequate understanding of the machines energy sources, lockout/tagout safety prop, and the means/methods of effectively isolating and locking out the machine.

The employee did not affix his own lockout device to the trim saws energy isolation device (ie disconnect). The employee was dependent on another persons lockout device.
