29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): Woodex Fly Ash Storage Building - On February 10, 2015, an employee was hospitalized for thermal burns on her lower legs after stepping into hot ashes. The employer did not assess the Woodex ash building for possible hazards to determine if personal protective equipment was required in that area.
