(b) On or about 02/11/15 - Utilities Mechanical Maintenance Shop, the Jet pedestal drill press did not have a chuck guard installed. (c) On or about 02/11/15 - Machine Shop, the IBARMIA pedestal drill press did not have a chuck guard installed. (d) On or about 02/11/15 - Electrical Shop, the Jet pedestal drill press did not have a chuck guard installed. (e) On or about 02/11/15 - Machine shop, several horizontal lathes did not have chuck guards installed.
