29 CFR 1926.102(a)(1): Eye and/or face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about February 10, 2015, Employees were using pneumatic nail guns to roof the residential independent living cottages under construction without eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:  On or about February 10, 2015, employees were roofing the residential independent living cottages under construction up to approximately 10 feet high without fall protection, exposing the employees to a fall hazard.
