29 CFR 1926.756(a)(1): During the final placing of solid web structural members, the load was released from the hoisting line before the members were secured with at least two bolts per connection, of the same size and strength as shown in the erection drawings, drawn up wrench-tight or the equivalent as specified by the project structural engineer of record: On or about February 10, 2015, during the installation of a structural member, an employee was exposed to a fall hazard of 14 feet to the lower level when the member was secured to a column using a loosened "C" clamp.

29 CFR 1926.1402(b): The equipment was assembled or used without ensuring that ground conditions were firm, drained, and graded to a sufficient extent so that, in conjunction (if necessary) with the use of supporting materials, the equipment manufacturer's specifications for adequate support and degree of the level of the equipment are met. On or about February 10, 2015, five employees were exposed to a struck by hazard when the outriggers on the crane were resting on a surface that did not adequately support the equipment.

29 CFR 1926.1417(d): The operator must not engage in any practice or activity that diverts his/her attention while actually engaged in operating the equipment, such as the use of cellular phones (other than when used for signal communications). On or about February 10, 2015, with a load suspended and the steel erectors connecting the beam, the operator got on the back of the boom truck, turned away from the controls with his back towards the work to orient his drawings to the way the beams and columns were being erected.
