29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:   (a)Quality Oilfield Services at 5023 WCR 6, Dacono, CO 80514:  On and before January 14th, 2015, the employer did not ensure that grounding and bonding methods to control electrostatic ignition sources when conducting pre-welding activities on a produced water tanker truck containing hydrocarbons and hydrocarbon vapors. This condition exposed an employee to explosion and fire hazards of flammable liquids and gases.   (b) Quality Oilfield Services at 5023 WCR 6, Dacono, CO 80514: On and before January 14th, 2015, the employer did not take adequate precautions to control ignition sources including heat and open flame, in that, a DeWalt polisher was used to polish a 3'' collar on the back of a produced water tanker truck containing hydrocarbons and hydrocarbon vapors which resulted in an explosion due to the ignition of vapors within the tank. This condition exposed an employee to explosion and fire hazards of flammable liquids and gases.
