29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) A & H Metals, Inc., 249 E. Chestnut Hill Rd., Newark, DE 19713 - The employer did not provide hardware necessary to lock electrical switches in the off position for all employees servicing machines such as hydraulic shears and brakes. Condition identified on or about February 13, 2015. Pursuant to 29 CFR 1903.19, the employer must submit abatement certification, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
