29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:   (a) On or about February 4, 2015, for the employees that fell approximately 20 feet to a lower level while performing drywall work activities on an adjustable aluminum scaffold plank where a 2x4 was nailed on edge as scaffold support at 220 South Minnesota Avenue, Sioux Falls, South Dakota.
