29 CFR 1910.242(a): The employer did not ensure that tools and equipment were in a safe condition:      a) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about February 10, 2015, during abrasive wheel grinding activities in the PV 12 department, the employer did not ensure that the Fein portable electric grinder, serial number 201007034882, was maintained in a safe condition in that general informational and warning labels on the unit were worn off/missing.     b) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about February 10, 2015, during abrasive wheel grinding activities in the PV 12 department , the employer did not ensure that abrasive wheels used on the Fein portable grinder, serial number 201007034882, were maintained in a safe condition in that the wheels had no labeling, etchings or other identifiers on them that would alert the user to critical information including but not limited to wheel type and wheel speed.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:   a) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about May 29, the employer did not keep the floor in the EDM production area orderly. The employer had a section of flooring that was not even with the remaining flooring, creating tripping hazards.
