29 CFR 1910.242(a): The employer did not ensure that tools and equipment were in a safe condition: a) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about February 10, 2015, during abrasive wheel grinding activities in the PV 12 department, the employer did not ensure that the Fein portable electric grinder, serial number 201007034882, was maintained in a safe condition in that general informational and warning labels on the unit were worn off/missing. b) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about February 10, 2015, during abrasive wheel grinding activities in the PV 12 department , the employer did not ensure that abrasive wheels used on the Fein portable grinder, serial number 201007034882, were maintained in a safe condition in that the wheels had no labeling, etchings or other identifiers on them that would alert the user to critical information including but not limited to wheel type and wheel speed.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: a) Job site, 500 Pleasant Valley Road, Trafford, PA: On or about May 20, 2015, the employer did not ensure that an appropriate annual inspection of the energy control procedure had been conducted in order to ensure that proper energy control practices were being followed and that compliance with this standard was being met. Examples of deficiencies found during the inspection included, but were not limited to, failing to use proper energy control procedures during blade changes on the Kasto Racine Horizontal band saw, not using appropriate energy control procedures during set up, parts flip and cleaning activities on the Gray Boring Mill & having affected level employees perform authorized level work activities under the energy control standard without proper energy control training.
