Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

In each case, any utility company which was involved was not notified in advance. a) Worksite, 21 Old Country Rd. Watermill NY - Employees engaged in demolition of gas piping did not ensure that the gas was turned off before the removal and cutting of piping was started; on or about 2/11/15. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
