Shaw Industries: On or about 2/17/2015 and at times prior there to, employees performed servicing/maintenance work on machines/equipment that include but were not limited to the KIA 80 vertical milling machine. The servicing and maintenance work included but was not limited to cleaning out metal chips/debris from the machine. The equipment was not locked out when employees bodily entered the machine to clean and remove metal/chips debris from inside the machine.

Shaw Industries: On or about 2/17/2015 and at times prior there to, employees performed servicing and maintenance work on machines/equipment that included but were not limited to vertical CNC milling machines, horizontal CNC milling machines, and the Reis Press. Periodic lockout/tagout inspections of the energy control procedures were not conducted with the authorized employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts and pinch points: A.) Shaw Industries: On or about 2/17/2015, employees were potentially exposed to an unguarded pinch point on the Kia 80 vertical milling machine. The pinch point was formed between the table and the edge of the machine when it was rotated by the operator to affix parts to the table.
